Wellness Screenings
The Office of Community Health offers residents the following FREE screenings:
- Diabetes
- Anemia
- Blood Pressure
- Heart Rate
- Body Fat Analysis
- Health Education
- Medication Review
Please call the office at (630) 483-5665, to schedule an appointment for a free screening. Please note that the above free screenings are for residents of Hanover Township. To check if you reside in our Township, visit here.
Additional Services
- Hemoglobin A1C - $20
- Total Cholesterol/Glucose - $10 (non-fasting, fingerstick)
- Complete Cholesterol - $25 (fasting, includes total cholesterol, HDL, LDL, triglycerides, and glucose)
- Vitamin D - $30
- TB Skin Testing - $20
- TB Blood Draw - $90
- PT/INR Monitoring - $10 (Patient must provide our office with an order from their physician prior to scheduling an appointment with us).
- Blood Draw Labs - Low cost lab services are available for individuals with no health insurance or who have private health insurance.*+ (Patient must provide our office with a lab order from their physician prior to scheduling an appointment with us).
