Home Visits

Hanover Township Nurses provide home visits for qualifying residents in need of health services.

Home visits are made for patients if:

• The resident is home-bound or unable to come in for an office appointment
• A home visit is requested by the resident’s physician
• A referral is made by a community partner

Nursing services provided in the home include:

• Blood draws/laboratory services
• PT/INR monitoring
• Injects
• Medication set up and management 
• Nurse evaluations and consultations
• Other qualifying services
