The mission of the Office of Community Health is to provide education and health promotion, prevent the spread of disease and illness, and to assist in accessing quality health services.

The Office of Community Health serves Hanover Township residents by providing affordable health-based services and programs that focus on preventative health screenings, improving overall health status, managing chronic diseases, navigating the health system and improving access to health services.

Some of the health screenings offered at our clinics include:

  • Screenings for diabetes
  • High blood pressure
  • Anemia
  • Cholesterol

We provide education on:

  • Chronic disease management
  • Nutrition
  • Medications
  • Healthy weight maintenance

Our Township nurses are also able to provide:

  • Lab services
  • Administer injections
  • Perform home visits for qualified home-bound residents (under a physician’s order)
  • CPR/AED  & First Aid classes and certification

We have two staff members who are Certified Application Counselors for the Affordable Care Act (Obamacare) and can assist residents in enrolling in health insurance coverage.
